Middle Georgia Heart offers remote care monitoring as a concierge service for patients with at least one ailment, making it a great service for those using chronic care management as well. Our in-house team of physicians, nurse practitioners, medical assistants, and licensed practical nurses directly monitor your health data for deviations that could be alarming. Patients participating in remote care monitoring will receive the direct phone number to the medical assistant or licensed practical nurse responsible for their case. We ease your mind by monitoring your health data for you, so we can manage and treat your condition and you can worry less.
No out of pocket expense for Medicare. All insurance accepted. Please contact individual insurance company for coverage information.
How Does Remote Care Monitoring Relate to Chronic Care Management?
Chronic care management is a care plan for patients with two or more ailments. Because remote care monitoring only requires patients to have one ailment to qualify for the program, any patient enrolled in (or eligible for) chronic care management is automatically eligible for remote care monitoring, as well.

How Remote Care Monitoring Works

Middle Georgia Heart is partnered with Remote Care Partners to offer remote care monitoring. Call our office at 478-207-5224 and a member of our team will walk you through the enrollment process over the phone.
Set Up SmartHub Device
The devices (scale, blood pressure cuff, and SmartHub) are sent directly to your door. The SmartHub is easy to set up – all you have to do is plug it into a wall outlet near a window and wait for the SmartHub to communicate that it is ready. The SmartHub does not require WiFi, as it connects to cellphone towers instead.
Remote Monitoring Begins!
Parameters are set based on your health needs, and if anything falls outside of those parameters, we will be alerted immediately. You can check your weight or blood pressure and the data is automatically sent to Middle Georgia Heart’s medical assistant or licensed practical nurse responsible for your remote care.
